ABSTRACT

Obesity and aging are two important epidemic factors for metabolic syndrome and many other health issues, which contribute to devastating diseases such as cardiovascular diseases, stroke and cancers. The brain plays a central role in controlling metabolic physiology in that it integrates information from other metabolic organs, sends regulatory projections and orchestrates the whole-body function. Emerging studies suggest that brain dysfunction in sensing various internal cues or processing external cues may have profound effects on metabolic and other physiological functions. This review highlights brain dysfunction linked to genetic mutations, sex, brain inflammation, microbiota, stress as causes for whole-body pathophysiology, arguing brain dysfunction as a root cause for the epidemic of aging and obesity-related disorders. We also speculate key issues that need to be addressed on how to reveal relevant brain dysfunction that underlines the development of these disorders and diseases in order to develop new treatment strategies against these health problems.

ABSTRACT

Idiopathic inflammatory myopathies are a group of rare but serious diseases. The treatment of refractory idiopathic inflammatory myopathy is always challenging, especially in children. Three cases of refractory idiopathic inflammatory myopathy treated by rituximab were reported and discussed with the review of relevant literature. All were female with on-set age of 8 years and 6 months, 11 years and 7 months, 4 years and 2 months old, respectively. All had acute onset, presenting with progressive and severe muscle weakness. All lost ambulation within 1 or 2 months, with difficult swallowing and low voice. Respiratory distress occurred in case 2 after an attack of asphyxia due to an aspiration of sputum, and ventilator support was required for 1 month. Rashes were detected at the initial stage of the disease in cases 2 and 3. Patient 2 showed facial erythematous papules, spreading to her neck and hands. Patient 3 showed purplish eyelids with peri-orbital swelling, generalized edema involving all her limbs. Creatine kinase (CK) levels were markedly elevated in all the patients, ranging from 6 000 IU/L to 28 819 IU/L. Anti-SRP antibody was identified in cases 1, and anti-NXP2 antibodies were confirmed in cases 2 and 3. MRI of both thighs in all the patients showed profound muscle and fascial edema. Muscle pathology of patient 1 showed prominent fiber variation and endomysial fibrosis, with overexpression of MHC-Ⅰ. While muscle pathology in patients 2 and 3 showed scattered fiber necrosis, regeneration, endomysial edema without inflammatory cell infiltration. All the patients were diagnosed with idiopathic inflammatory myopathy and failed to the initial treatment including adequate glucocorticoids and high-dose immunoglobulin therapy. Other immunosuppressants (methotrexate, cyclophosphamide) were also tried in cases 2 and 3 with poor response. Then all the patients were treated with rituximab combined with glucocorticoids. Patient 1 regained normal strength and discontinued rituximab at the end of her last follow-up (2 years and 7 mouths). Though calcinosis developed during the follow-up period, significant improvement was noticed in cases 2 and 3 (both regained the ability to walk independently) at the end of their last follow-up after 2 years and 8 months, 3 years and 2 months respectively. Long-term rituximab therapy may improve the prognosis of refractory idiopathic inflammatory myopathy, especially with positive anti-SRP and anti-NXP2 antibodies.

ABSTRACT

To develop a fluorescence polarization (FP)-based high-throughput screening (HTS) assay to identify novel small-molecule antagonists targeting β-catenin/TCF4 (T-cell factor 4) interaction, recombinant human β-catenin was expressed in Escherichia coli Rosetta (DE3) cells and purified by HisTrapTM column. The bioactivity of purified β-catenin was further analyzed by enzyme-linked immunosorbent assay (ELISA). According to FP principle, the β-catenin/TCF4 binding model was performed, and fluorescence isothiocyanate (FITC) labeled TCF4 peptide (FITC-TCF4) served as the molecular probe of adaptor for binding to β-catenin. The FITC-TCF4 and β-catenin working concentration were optimized, and the binding conditions (complex stability and dimethylsulfoxide (DMSO) tolerance) have been investigated yet for further hits screening. The results showed that recombinant human β-catenin was successfully expressed and purified β-catenin exhibited favorable bioactivity in ELISA binding assay. Subsequently, the FP-based HTS assay was performed using 20 nmol·L-1 FITC-TCF4 and 100 nmol·L-1 β-catenin. Under these optimized conditions, a high Z´factor of 0.88 was achieved in a 384-well format and this FP-based HTS assay was very stable with regard to DMSO. Through screening of a natural-based product library (NBPL) using the established FP-based HTS assay, three hits (sanguinarine, chelerythrine, and compound S720) were identified as potential β-catenin/TCF4 interaction antagonists. Taken together, we have successfully developed a simple, robust and reliable FP-based HTS assay for screening of novel antagonists targeting β-catenin/TCF4 interaction.

ABSTRACT

In the tumor microenvironment, the interaction of programmed cell death protein 1 (PD-1) and programmed death-ligand 1 (PD-L1) can promote the immune escape of tumor cells. The application of PD-1/PD-L1 blockade can restore host immune function to efficiently identify and kill tumor cells. In recent years, PD-1/PD-L1 blockade has shown remarkable clinical effects in a variety of tumors and U.S. Food and Drug Administration has approved nivolumab and pembrolizumab for the treatment of unresectable/metastatic melanoma, non-small cell lung cancer and etc. Hence, a growing number of clinical trials on PD-1/PD-L1 blockade for advanced gastric cancer have been carried out, and the overall survival and objective response rate of most trials are promising. This paper reviewed PD-1/PD-L1 signaling pathway, the relationship between the expression of PD-L1 and the classification and prognosis of gastric cancer, and the clinical results of PD-1/PD-L1 blockade in advanced gastric cancer.

ABSTRACT

Objective To investigate the effect of hepatitis B virus X (HBx)protein on the apoptosis of placental trophoblastic cells and its potential mechanism.Methods A pcDNA3.1 expression vector of HBx gene was constructed and transfected into JEG-3 and HTR-8 human placental trophoblastic cell lines,respectively.After transfection for 48 h,RT-PCR and immunofluorescence analyses were made to detect HBx mRNA and protein expressions.Flow cytometry was used to detect the early apoptosis status of JEG-3 and HTR-8 cells.The expressions of PI3K and p-Akt were detected by immunofluorescence and Western blotting.Results After transfection for 48 h,RT-PCR and immunofluorescence analyses showed that HBx mRNA and protein expressions were detected in JEG-3 and HTR-8 cells.Flow cytometry revealed that early apoptosis of JEG-3 and HTR-8 cells was reduced by pcDNA-HBx transfection (P <0.05).Immunofluorescence and Western blotting showed that PI3K and p-Akt were significantly upregulated in HTR-8 cells (P < 0.05 ).Conclusion HBx gene can be transfected into JEG-3 and HTR-8 human placental trophoblastic cell lines,respectively.After the transfection,the early apoptosis of JEG-3 and HTR-8 cells is reduced.Its inhibition on apoptosis is related to the activation of the PI3K/Akt signaling path-way.

Objective:To observe whether pretreatment with Pam3CSK4,a TLR2 agonist,could decrease the inflammation response in kidney from mice with systemic MRSA infection,and to investigate the mechanism of the attenuation of inflammation with Pam3CSK4 pretreatment. Methods:BALB/c mice were pretreated with Pam3CSK4 (10 μg/100 μl/each mouse) or PBS via tail vein once daily for two consecutive days. All mice were infected with live MRSA (ATCC43300) at 2×107 CFU/each mouse (via tail vein) 24 h after the second treatment. The levels of cytokines in kidney were measured by ELISA and real-time PCR,respectively. The relative expression of TLR2,IRAKs etc. were detected by real-time PCR. Western blot was performed to detect the phosphorylation of NF-κB, the expression of IRAK-M and A20,respectively. Results:The level of TNF-α,IL-6,IL-1β,CCL3 and IFN-γ in renal tissue from mice pretreated with Pam3CSK4 was decreased significantly compared with that from PBS-treated mice,respectively. Pam3CSK4 pretreatment down-regulated the relative expression of TLR2, inhibited the expression of IRAK-1 and the phosphorylation of NF-κB post infection. The expression of IRAK-M,one of the negative regulators in TLRs signaling pathway was increased significantly in renal tissue from Pam3CSK4-treated mice post infection. Conclusion:Pam3CSK4 pretreatment attenuated the inflammation response in kidney from mice with systemic MRSA infection,and these attenuation is related with up-regulation of IRAK-M.

ABSTRACT

<p><b>OBJECTIVE</b>Noncystic fibrosis (non-CF) bronchiectasis remains as a common health problem in Asia. Pathogens' distribution in airways of patients with non-CF bronchiectasis is important for doctors to make right decision.</p><p><b>DATA SOURCES</b>We performed this systematic review on the English language literatures from 1966 to July 2014, using various search terms included "pathogens" or "bacteria" or "microbiology" and "bronchiectasis" or "non-cystic fibrosis bronchiectasis" or "non-CF bronchiectasis" or "NCFB."</p><p><b>STUDY SELECTION</b>We included studies of patients with the confirmed non-CF bronchiectasis for which culture methods were required to sputum or bronchoalveolar lavage fluid (BALF). Weighted mean isolation rates for Haemophilus influenzae, Pseudomonas aeruginosa, Streptococcus pneumoniae, Stapylococcus aureus, Moxarella catarrhails were compared according to different methodology.</p><p><b>RESULTS</b>The total mean bacterial culture positive rates were 63%. For studies using sputum samples, the mean positive culture rates were 74%. For studies using BALF alone or BALF and sputum, it was 48%. The distributions of main bacterial strains were 29% for H. influenzae, 28% for P. aeruginosa, 11% for S. pneumoniae, 12% for S. aureus, and 8% for M. catarrhails with methodology of sputum. Meanwhile, the bacterial distributions were 37% for H. influenzae, 8% for P. aeruginosa, 14% for S. pneumoniae, 5% for S. aureus, and 10% for M. catarrhails with methodology of BALF alone or BALF and sputum. Analysis of the effect of different methodology on the isolation rates revealed some statistically significant differences.</p><p><b>CONCLUSIONS</b>H. influenzae accounted for the highest percentage in different methodology. Our results suggested that the total positive culture rates and the proportion of P. aeruginosa from sputum and BALF specimens had significant differences, which can be used in further appropriate recommendations for the treatment of non-CF bronchiectasis.</p>

ABSTRACT

Objective To construct the three-dimensional (3D) fluid model at the physiological level of shear stresses and study the effects of fluid shear stress (FSS) on adhesion, differentiation and mechanical sensitivity of osteoblasts. Methods The MC3T3-E1 osteoblasts cultured on β-tricalcium phosphate (β-TCP) scaffolds were subjected to various FSSs in the perfusion flow chamber for 6 hours to compare cell adhesion in FSS-loading groups and control group. Nitric oxide (NO) and alkaline phosphatase (ALP) were detected to compare mechanical sensitivity and cell differentiation. The FSS magnitude and distributions corresponding to various fluid rates were calculated with nonlinear fluid-structure coupling analysis. Results Cell adhesion rate was up to 74%~81% when the average FSS magnitude was lower than 0.4 Pa, but reduced to 60.22% when the average FSS was 0.41 Pa. The NO production rate reached the maximal concentration after loading for 5 min, then significantly reduced at 15 min, and gradually diminished to none at 30 min. ALP level significantly increased (P0.05) with the increase of shear stress. Conclusions Majority of the cells kept a normal adherence to the scaffold at the physiological level of shear stresses. The mechanical sensitivity of the cells under 3D condition was dependent on the FSS rate, which was consistent with two-dimensional (2D) condition. When the average FSS was lower than 0.304 Pa in the scaffold, FSS could significantly promote cell differentiation, but no significant change in cell differentiation could be found when FSS was higher than 0.304 Pa. The present study is expected to accelerate the realization of bone tissue engineering.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect difference of behavior training with head needling retention and behavior training after acupuncture for autism children.</p><p><b>METHODS</b>Sixty qualified autism children were divided randomly into simultaneous head needling retention and behavior training group (trial group) and behavior training after acupuncture treatment group (control group) with 30 case in each group. Retention needles on the head with simultaneous behavior training was applied for the trial group. The main acupoints included Sishen Xue, Dingshen Sanxue (3 points for mental tranquilization), Nao Sanxue (3 points for the function of brain), Shou Zhisanxue (3 points for mental activities on hand) and Zozhi Sonxue (3 points for mental activities on foot). Other points were combined according to conditions of patients. Needles on the 4 extremities were withdrawn first after 30 minutes, needles on head were remained during behavior training. While behavior training was applied to the control group when acupuncture treatment was completely accomplished. Treatments were applied once a day to both groups. And 3 months was taken as one observation cycle. Estimation was made on therapeutic effect and developing level of autism children with CARS and PEP.</p><p><b>RESULTS</b>The total effective rate of the trial group was 83.3% (25/30), better than 66.7% (20/30) of the control group (P < 0.05). The CARS scores of both groups declined after the treatment. And the score of trail group was lower than the control group (all P < 0.05). While the PEP scores of both groups increased, and the score of trail group was higher than the control group (all P < 0.05). The increasing level of scores of cognitive understanding and cognitive expression were all better than the control group (all P < 0.05).</p><p><b>CONCLUSION</b>The effect of behavior training with head needle retention on autism children is better than behavior training after acupuncture treatment, especially in enhancing cognition understanding and cognition expression.</p>

ABSTRACT

Objective To observe the effect of endogenous heme oxygenase 1 (HO-1)/carbon monoxide(CO)system on brain damage induced by recurrent febrile seizures (FS) through GRP78-PERK-eIF2α-CHOP signal pathway.Methods Forty-eight SD male rats of 21 days were randomly divided into 4 groups:control group,FS group,FS with ZnPP-Ⅸ (FS + ZnPP-Ⅸ) group,and FS with Hemin(FS + Hemin) group,12 rats in each group.Hyperthermia treatment was carried out 10 times,once every 2 days.For group FS + Hemin,Hemin,a CO donor was administered intraperitoneally 0.5 hour before each immersion.For group FS + ZnPP-Ⅸ,ZnPP-Ⅸ,an inhibitor of HO-1 were preconditioned intraperitoneally 0.5 hour before each immersion.The seizure latency,seizure intensity and seizure duration of FS rats were recorded.The content of CO in cortex was measured by using dual wavelength spectropotometey.Neuron apoptosis was detected by using TUNEL.The expressions of GRP78,p-PERK,p-eIF2α and CHOP in cortex were measured by using immunohistochemistry.Results No difference in the seizure latency,seizure intensity and seizure duration in the rats FS + ZnPP-Ⅸ group or the FS + Hemin were recorded,compared with those in the FS group(all P >0.05).The content of CO in cortex increased after recurrent FS (P < 0.05).The generation of CO further up-regulated the expressions in FS + Hemin group,while it decreased the expressions in FS + ZnPP-Ⅸ group(all P <0.05).Neuronal apoptosis was evident in the FS group.Hemin reduced neuronal apoptosis,up-regulated the expression of GRP78,down-regulated the contents of CHOP,p-PERK and p-eIF2α ;ZnPP-Ⅸ further induced neuronal apoptosis,and down-regulated the expression of GRP78;it aslo up-regulated the expression of CHOP,p-PERK and p-eIF2α.Conclusions Endogenous HO-1/CO system protects the rats from brain damage induced by recurrent FS through endoplasmic GRP78-PERK-eIF2α-CHOP/GADD153 signal pathway.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the quality of life in patients with gout and their influencing factors, and to observe the effects of the intervention program of stage-based treatment of integrative medicine (IM).</p><p><b>METHODS</b>Totally 120 patients with acute attack of gout within 72 h were randomly assigned to the treatment group and the control group, 60 in each group. Patients in the treatment group were treated with Huzhang Tongfeng Granule (HTG), diclofenac sodium extended-release capsule and Jinhuang Ointment (JO) in the acute stage, and Yinlian Tongfeng Granule (YTG) and Benzbromarone Tablet (BT) in the intermission stage. Patients in the control group were treated with diclofenac sodium extended-release capsule in the acute stage, and BT in the intermission stage. All patients were treated for 12 weeks. The quality of life (QOL) before and after treatment was investigated by questionnaire.</p><p><b>RESULTS</b>Before treatment there were no statistical difference in the physiological function, psychological function, social function, health self-awareness and total score between the two groups (P > 0.05). After treatment the scores of the four aspects and the total score were significantly improved in the two groups (P < 0.01). And the improvement of the treatment group was better than that of the control group (P < 0.01). There was no statistical difference in the gender, age, marital status, educational level, QOL with or without associated disease between the two groups (P > 0.05). The QOL of patients with joint stiffness or deformity was less than that of those without joint stiffness or deformity (P < 0.01). The total QOL scores of the gout patients were obviously correlated with the course of diseases (r = -0.324, P < 0.01).</p><p><b>CONCLUSIONS</b>The QOL of patients with gout was correlated with the course of disease and joint stiffness or deformity. Stage-based treatment of IM could significantly improve the QOL of f out patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the surface roughness property of the titanium castings cast in a new investment for titanium casting.</p><p><b>METHODS</b>Six wax patterns (20 mm × 20 mm × 0.5 mm) were invested using two investments: three in a new titanium investment material and three in the control material (Rematitan Plus). Six titanium specimens were obtained by conventional casting. After casting, surface roughness of the specimens were evaluated with a surface profilometer.</p><p><b>RESULTS</b>The surface roughness of the specimens cast in new titanium investment material was (1.72 ± 0.08) µm, which was much smaller than that from Rematitan Plus [(1.91 ± 0.15) µm, P < 0.05].</p><p><b>CONCLUSIONS</b>The surfaces of titanium cast using these two investment materials are both smooth enough to fulfill the demand of the titanium precision-casting for prosthodontic clinical use.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effect and its mechanism of acupoint catgut-embedding for experimental colitis in rats.</p><p><b>METHODS</b>Eighteen SD rats were randomly divided into a normal control (NC) group, a model (MO) group and a acupoint catgut-embedding (AC) group, 6 rats in each group. Animals in MO group and AC group were treated by trinitro-benzene-sulfonic acid (TNBS) to establish model with colitis. No other treatment was given to the rats in MO group, but acupoint catgut-embedding was implanted at "Shangjuxu" (ST 37), "Tianshu" (ST 25) and "Dachangshu" (BL 25) in the rats in AC group. The symptoms of diarrhea and bloody stool, and histopathology changes in colon were observed 15 days after the treatment. Expression of IL-17, beta2AR, NFkappaBp65 were observed by immunohistochemistry. Expressions of NF-kappaBp65 and beta2AR in splenic lymphocyte were detected by the Western blot method.</p><p><b>RESULTS</b>Diarrhea and mucus bloody purulent stool were soon controlled, and colon mucosa injures were obviously improved in AC group. The NF-kappaBp65 value in splenic lymphocytes of 249.70 +/- 13.66 in MO) group was higher than 86.22 +/- 8.09 in NC group (P < 0.01), and 219.02 +/- 7.42 in AC group was less than that in MO group (P < 0.01). The expression of beta2AR in splenic lymphocytes of 594.97 +/- 173.22 in MO group was less than 957.45 +/- 171.56 in NC group (P < 0.01), and 1335.93 +/- 244.34 in AC group was higher than that in MO group (P < 0.01). The expression of IL-17 in colon mucosa in MO group was increased, while the expression of IL-17 in colon mucosa in AC group was decreased.</p><p><b>CONCLUSION</b>Acupoint catgut-embedding at Shangjuxu (ST 37), Tianshu (ST 25) and Dachangshu (BL 25) has obviously effect in treating experimental colitis and the mechanism may be related to regulate the expression of IL-17, beta2AR and NF-kappaBp65.</p>

ABSTRACT

Congenital long QT syndrome ( LQTS) is a cardiac ion channel dysfunction, leading to prolonged myocardial repolarization time. It is characterized by the typical ECG QT interval prolongation and torsades de pointes. It shows clinical recurrence of cardiogenic syncope and even lead to sudden death. Molecular genetic studies have revealed a total of 12 forms of congenital LQTS caused by mutations in genes of the potassium, sodium and calcium channels or membrane adapter located on chromosomes 3, 4, 7, 11, 12, 17, 20 and 21. This review summarized the studies of the pathogenesis of LQTS and gene-related treatments.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the characteristics of and relationship between occlusal plane and masticatory path in young adults with individual normal occlusion, and to provide clinical implications on both morphological and functional reference of occlusal plane location.</p><p><b>METHODS</b>Forty-three young adults with individual normal occlusion were included. Lateral projections of cephalometric radiographs were taken. The parameters of anatomical landmarks were analyzed. The BioEGN mandibular kinesiography analyzer was used to record the path of incisal edge of mandibular central incisor movement during mastication. The Pearson's linear correlation analysis was performed to analyze the relationship of inclination of occlusal plane and data mentioned above with SPSS13.0 software.</p><p><b>RESULTS</b>The inclination of occlusal plane (the angle between occlusal plane and Camper's line OP-CP) was (2.1 ± 3.8)° in youth with normal occlusion. Means of maximal distances of 3-dimensional masticatory mandibular movement paths were as follow: the vertical length was (20.22 ± 4.28) mm, the anterior-posterior width was (11.42 ± 2.45) mm, and the lateral width was (8.62 ± 1.52) mm. The inclination of occlusal plane was negatively correlated to the lateral width of masticatory path in frontal plane (r = -0.39, P < 0.05). Occlusal plane was approximately vertical to the opening and closing path in the vicinity of intercuspal position during mastication. The inclination of occlusal plane was positively related to occlusal plane-closing angles in the upper segments of closing path during mastication (P < 0.05). Masticatory axis angle was (66.50 ± 7.98)° with no significant correlation to the inclination of occlusal plane (P > 0.05).</p><p><b>CONCLUSIONS</b>These results suggest that the sagittal inclination of occlusal plane can influence 3-dimensional morphology of masticatory path.The inclination of occlusal plane has a guiding effect on masticatory movement in the upper segments of closing path.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To probe the mechanism of electroacupuncture at points "Weibingfang" in treatment of acute gastric mucosal lesion.</p><p><b>METHODS</b>Forty Wistar rats of sanitary degree were randomly divided into 4 groups, normal group, model group, sham-model group and Weibingfang group, 10 rats in each group. The acute gastric mucosal lesion model was made by intragastric perfusion of anhydrous alcohol. The Weibingfang group were treated by electroacupuncture at "Neiguan" (PC 6), "Zhongwan" (CV 12) and "Zusanli" (ST 36) with sparse-dense wave, frequency of 10-30 Hz, current intensity of 2 mA, for 20 min. One hour after the treatment, the blood from the abdominal aorta and the gastric mucosa tissue were taken, and serum epidermal growth factor (EGF) level and epidermic growth factor receptor mRNA (EGFR mRNA) expression in the gastric mucosa were detected with enzyme linked immunosorbent assay (ELISA) and reverse transcriptase-polymerase chain reaction (RT-PCR) respectively.</p><p><b>RESULTS</b>There were significant differences in blood EGF level (41.62 +/- 12.58) ng/L and EGFR mRNA expression (0.78 +/- 0.03) in the model group were significantly different from [(60.37 +/- 12.01) ng/L and 0.55 +/- 0.04] in the normal group and [(61.21 +/- 13.46) ng/L and 0.53 +/- 0.05] in the sham-model group (P < 0.05); after electroacupuncture, blood EGF level (70.59 +/- 10.14) ng/L increased and the EGFR mRNA expression (1.18 +/- 0.02) in the gastric mucosa was up-regulated with a significant differences as compared with those in the model group (P < 0.05).</p><p><b>CONCLUSION</b>Point "Weibingfang" can promote proliferation, differentiation and migration of the gastric mucosal epidermic cells to repair the gastric mucosal lesion.</p>
